ONLINE Screening LIVE
Submitted By Jen Stewart on Sun 10 25, 20
Hi Hornets Families,

Effective Monday Oct 26, all teams will be responsible for screening their team on entry to the arena.

Team staff or a parent volunteer for your team will be checking that you have completed your online COVID SCREENING prior to entry

SCREENING IS NOT OPTIONAL - this must be done every time your team enters the arena.

SCREENING is MANDATED by the OMHA, PUBLIC HEALTH and ASPHODEL NORWOOD COMMUNITY CENTRE.The online COVID SCREENING form has been added to our website.

The online COVID SCREENING form has been added to our website.

Families are asked to complete the online form about one hour before your teams practice or game.

The form is to include the player going on the ice and the name of the parent/guardian that is entering the arena.

1 parent/guardian is allowed entry per player. Siblings are not permitted to enter.

For U15 and u18 players attending without a parent, they will still be expected to show a copy of the completed screening form.

Screeners are to confirm that parents have completed their online form - the form is set with an auto-email reply that can be shown to screener.

Coaches/Screeners may choose to have their team email them or screen shot their completed form to them prior to arrival at the arena. Please check with your coach/manager on this detail.

Should you be unable to provide proof that you have completed online - you will be asked to review the screening questions that can be found at the arena entrance. If you can answer no to all and pass you will be permitted entry. Screeners will record names and accompanying adult, and log into the online form for them.

NDMS needs your support in enforcing the rules and there are no exceptions or “just this times”.  Teams caught not completing the appropriate screening or breaking the rules regarding attendance at the arena will be forced to miss games and practices and coaches will receive suspensions.

Failure to comply to the rules of our ice contract, OMHA and the rules set forth by the Public Health unit could result in a closure of our programs, and we are working very hard to ensure this doesn’t happen but we need your help.

Summary of key points

- All families need to complete ONLINE COVID SCREENING prior to each time on ice.
- Screen shot your entry, it is set with an email auto reply
- Any parent or player reporting yes to any symptoms is not permitted to enter
- 1 parent/guardian per player U13 and under
- No siblings are permitted entry
- Absolutely NO IN & OUT privileges - please ensure you have ALL BELONGINGS (water bottles, shoes, phones, purses, coats etc) with you.

We realize that these rules are difficult but we must ask that everyone follow these rules.  Failure to do so could have consequences for the entire organization.  The rules are part of our ice rental agreement with the municipality, and although we are hopeful that the rules will be relaxed in the weeks and months ahead, we must enforce what is in our current contract.

We ask that everyone do their part!  If you need support to get your player on and off the ice, please don’t hesitate to speak with your coach or any member of the NDMS executive.

Sincerely,

Jennifer Barton-Crowley
NDMS President
